A 35-week PreMiEr study analysing nearly 2,000 samples from 30 hospital rooms found that enhanced antimicrobial cleaning of sink drains reduced dangerous pathogen-associated bacteria through ecological replacement rather than sterilization. Treated drains lost roughly half their bacterial diversity and were colonised by environmental bacteria that appeared more resistant to pathogen recolonisation.
Critically, genera containing ESKAPEE pathogens were far more abundant in untreated sinks. The intervention had no measurable effect on nearby countertop surfaces, challenging traditional sterilization-focused approaches and suggesting that managing microbial community structure may be a more realistic long-term goal for controlling sink-related transmission risks.
